You must submit the following documents to apply for a TUE:

1. TUE Application Form

• To be completed by the athlete and his/her physician.  icon-arrow-circle-down TUE Application Form

2. Detailed medical report and/or letter

• Confirming the diagnosis by your physician (including medical history, laboratory results, examination reports, etc.)
• Download and print out the <Medical Information to support the decisions of TUECS – also called TUE Physician Guidelines> corresponding to your medical condition at  icon-external-link World Anti-Doping Agency’s (WADA) webpage
• Show the Guidelines to your physician and ask him/her to provide the diagnostic reports according to requirement as stipulated in “Section 4: Diagnosis”
